FORMS AND TEMPLATES – FORMS OTHER THAN THE FORMS FOR ELECTRONIC COMPLETION MADE AVAILABLE WILL NOT BE ACCEPTED (SENDING FORMS WITH TEXT CONVERSIONS TO PDF AND VICE-VERSA, SCANNED FORMS FILLED OUT BY HAND, FORMS SAVED AS IMAGE OR ANY OTHER FORMS AMENDMENT OF THE FORM FOR ELECTRONIC COMPLETION IN PDF WILL HAVE THE CONSEQUENCE OF NON-APPROVAL OF THE REGISTRATION)

If you are having problems using the electronic form option, please try to resolve them before the registration deadline.

AFTER THESE ACTIONS, ATTACH THE FILES IN YOUR REGISTRATION.
FORMS AND MODELS: – FORMS WITH FORMATTING CHANGED BY CONVERTING PDF TO TEXT, SCAN THAT ARE NOT CLEAR AND ALIGNED OR FILES IN FORMATS OTHER THAN PDF WILL NOT BE ACCEPTED
FORM 1 – APPLICATION
Fill out electronically form, sign (this form accepts the insertion of an image file for signature – png or jpg file with signature) and save as a PDF, name the file according to the instructions in the selection notice (form-1-PPGCN-2024-1-full name ) . Do not use any altered, scanned or save as an image other than the electronic form made available for registration – see instructions above!!!
FORM – REFERENCE LETTER
Candidates must send three reference letters signed by people related to candidate’s University Education or Professional Activities. Choose of of the form options below, fill in the preliminary information regarding your application, save the file in pdf naming the file as: Reference letter – PPGCN-2024-1 Candidate full name. Forward it to the declarant requesting the completion and submission to the e-mail: pgcn@uenf.br. Letters receveid in the PPGCN e-mail after the application deadline will not be considered.
– Option 1: Print, fill in the preliminary informations in legible handwriting, scan, and save in pdf naming the file and proceeding as instructed above.
– Opção 2: Fill in the preliminary informations and save in pdf naming the file and proceeding as instructed above (this form accept insertion of an image file – png or jpg – with signature)
ANNEX 3 (MASTER CANDIDATES) AND ANNEX 4 (DOCTORATE CANDIDATES)
In the evaluation, filling out the candidate’s self-assessment and the correct ordering of the evidential documents will be observed in accordance with items 8.1.2.1 and 8.1.2.2.(Master Level) and 8.2.2.1 and 8.2.2.2 (Doctorate Level). Only items that have been proven and recorded in the Lattes Curriculum or equivalent for foreign candidates will be considered.
ANNEX 3 (MASTER)
Fill out electronically and save in PDF, naming the file as Annex 3-PPGCN-2024-1-full name. Proceed in accordance with the instructions in items 2.2.2.1.3 (documentation 2) and 6.3.1 of the notice.
ANNEX 4 (DOCTORATE)
Fill out electronically and save in PDF, naming the file as Annex 4-PPGCN-2024-1-full name. Proceed in accordance with the instructions in items 2.2.2.1.3 (documentation 2) and 6.3.1 of the notice..
